Cook was last seen by her husband at their residence in the vicinity of the 8200 block of west Indian School Road in Phoenix, Arizona on December 14, 1996. She has never been heard from again. Cook may have been involved with illegal drugs at the time of her disappearance. Few details are available in her case.
